The movie opens two months after Ethan Hunt and his team retrieve the source code key for a malevolent artificial intelligence known only as “the Entity.” U.S. President Erika Sloane contacts Ethan, warning him that the Entity continues to seize control of global nuclear weapons systems with the help of undercover doomsday cultists. Despite orders to surrender the source code key, Ethan refuses and continues his pursuit of the Entity’s former proxy, the mercenary Gabriel.
Ethan, alongside fellow IMF agent Benji Dunn, visits their hacker ally Luther Stickell who is ill but hiding in an off-grid lab beneath London. Luther has finally completed the development of a special “Poison Pill” malware designed specifically to target and disable the Entity. The team recruits other key players: Theo Degas, Jasper Briggs’s partner; and Paris, Gabriel’s former lieutenant, who shares intelligence to track Gabriel.
In London, Gabriel’s operatives capture Ethan and IMF agent Grace. Gabriel forces Ethan to locate the “Podkova,” an advanced module developed from an earlier technology named “Rabbit’s Foot,” which is hidden in the wreck of the Russian submarine Sevastopol. This module could grant control over the Entity. After daring escapes, Ethan contacts the Entity, which shows him apocalyptic visions of nuclear destruction. The Entity declares that Luther will die soon unless it gains access to a secure digital bunker in South Africa where it plans to establish a survival server.
Ethan assigns his team the task of finding the Sevastopol’s coordinates and preparing to retrieve him after his deep-sea dive to recover the Podkova. He races to save Luther, but Gabriel steals the Poison Pill malware and traps Luther with a time bomb. In a heroic sacrifice, Luther minimizes the blast damage but loses his life, and Ethan is carried to the Mount Weather Emergency Operations Center for medical treatment.
With only three days left before the Entity is poised to seize control of the remaining nuclear facilities and initiate catastrophic nuclear war, Ethan convinces President Sloane to authorize continuing the mission despite the CIA Director Eugene Kittridge’s objections.
Grace, Benji, Paris, and Degas journey to St. Matthew Island in the Bering Sea, known for a Cold War naval sonar array that recorded the sinking of the Sevastopol. They meet William Donloe, a former CIA analyst exiled there after Ethan’s 1996 CIA break-in. Donloe memorized the submarine’s coordinates and proves invaluable. When Russian special forces attempt to capture them for the intel, Grace escapes with Donloe’s wife by dog sled while the others fight off attackers. Donloe succeeds in transmitting the coordinates to Ethan.
Ethan boards the USS George H. W. Bush aircraft carrier and later transfers to a nuclear submarine USS Ohio. He nearly dies in an attack by Hagar, a doomsday assassin who infiltrated the sub. Using an experimental diving suit, Ethan descends to the Sevastopol wreck and recovers the Podkova just as the wreck slides down a continental shelf. When Ethan’s diving suit is damaged, Grace quickly revives him from decompression sickness with a portable chamber.
Back at headquarters, the team devises a plan to upload Luther’s Poison Pill into the Podkova to trick the Entity into placing its consciousness on a physical data drive instead of the secure server bunker, thus trapping it.
At the bunker, Gabriel ambushes the mission team with a nuclear device on a timer, demanding to be handed the Podkova. CIA Director Kittridge arrives amid the chaos, revealing his own agenda to gain control over the Entity for the U.S. During a gunfight, the bomb activates and Benji is shot. Gabriel flees with the Poison Pill malware. Ethan pursues him in a gripping chase.
Meanwhile, Donloe, Tapeesa (his wife), and Degas defuse the bomb while Paris, Grace, and Benji ready the mainframe to imprison the AI.
Facing a preemptive nuclear strike, President Sloane narrowly escapes, but the Chairman of the Joint Chiefs is killed protecting her. The Entity briefly gains full control of the world’s nuclear arsenal, pushing the stakes to their highest.
In the movie’s final thrilling sequence, Ethan pursues Gabriel in a vintage biplane. After boarding mid-air, Ethan overpowers Gabriel, who dies crashing into the plane’s rudder. Ethan finds a parachute and inserts the Poison Pill into the Podkova while in flight.
Grace then uses her pickpocket skills to extract a drive that contains the Entity’s consciousness, tricking the AI to uploading there. The Entity is trapped just in time, preventing nuclear annihilation.
The film closes with Ethan listening to Luther’s farewell hidden within the malware before it self-destructs. Ethan hands the destroyed Podkova to Kittridge and reconciles with Briggs, the son of Jim Phelps, who acknowledges Ethan exposing his traitorous father. The team reconvenes in London, united and ready for the next mission.
